Client Intake Form

Attorney-Client Relationship. You understand and agree that filling out this form does not create an attorney-client relationship and that such a relationship is only established if The Stanfield & Dupre Law Firm agrees to represent you in your legal matter. You understand and agree that The Stanfield & Dupre Law Firm may require you to enter into an Engagement Agreement prior to beginning work on your matter.
Initial Consult. The initial 1 hour consult is free of charge.  You agree that all time spent after the initial consult will be billed at The Stanfield & Dupre Law Firm’s hourly rate.
Attorney Fees. You understand and agree that The Stanfield & Dupre Law Firm charges for its services on an hourly basis unless otherwise agreed in writing. You agree that you have been informed of hourly rate and consent to being charged for services rendered by an attorney based on that hourly rate. You understand and agree that invoices will be sent to you at least once per month via email or in the mail and you agree to pay said invoices within 10 days of receipt unless otherwise agreed in writing. You understand that The Stanfield & Dupre Law Firm may require you to provide a retainer before beginning work on your case.
External Services & Costs. You understand and agree that The Stanfield & Dupre Law Firm will bill all external services [e.g. courier services, court reporters, videographers, expert witnesses] at the rate then in effect for the provider at the time the services are incurred. You understand and agree that we may incur costs on your behalf. Typical costs include, for example, photocopy, court filing fees, deposition expenses, service of process, witness fees, transportation, automobile mileage expense, and fees for experts’ reports, consultation and testimony. You authorize us to expend those costs that we deem reasonably necessary.
